Case management & coordination

I work across your services, from health and disability to housing, education and Centrelink. With your consent, I'll talk to the people involved, arrange care team meetings and keep everyone on the same page.

Advocacy & system navigation

Help to understand and deal with the NDIS, health, housing, Centrelink and other systems, and to make sure your voice is heard.

Counselling & emotional support

Talk through stress, grief, loss, life changes and relationship difficulties with someone who listens and sees the bigger picture.

NDIS capacity building

Build skills for daily living, relationships and community participation. [Relevant support categories, e.g. Improved Daily Living, Improved Relationships]

Family & carer support

Help for families and carers to understand needs, manage stress, communicate with services and look after themselves too.

Assessments & reports

Psychosocial assessments and reports that capture your whole situation, for NDIS plan reviews, access requests and other services.

Complex situations

When things haven't worked before

Some situations are complicated: several issues overlapping, lots of services involved, or support that has been tried and hasn't stuck. That doesn't mean nothing can change.

I start by finding out what's already been tried, what helped, what didn't and why. Then we look for a fresh approach together, rather than repeating what's failed.

FAQ

Common questions

Will you talk to my other providers?

Yes, if you'd like me to. With your written consent, I'll contact your GP, therapists, support coordinator, school, housing provider or other services to share information and coordinate support. You decide who I speak to and what is shared.

I've tried other services and it didn't work. Why would this be different?

I can't promise a quick fix, but I won't just repeat what's already been tried. We start by looking at what has and hasn't worked, and why. Because social work looks at your whole situation, I can often spot gaps or connections that a single-focus service might miss.

I'm a professional. Can I refer someone?

Yes. Support coordinators, plan managers, GPs, allied health professionals and other services are welcome to get in touch through the contact page.

Do I need a referral?

No, you can contact me directly. [Update if you offer Medicare-rebated sessions, which need a GP referral and Mental Health Treatment Plan.]

Where do sessions happen?

I see people face-to-face across Gippsland, Victoria [e.g. in your home, in the community or at ____]. If you live anywhere else in Australia, we can work together by telehealth.

What is your cancellation policy?

[e.g. Please give at least 48 hours' notice. NDIS short-notice cancellations are charged in line with NDIS rules.]

Not sure if I can help?

I'm happy to talk through your situation before you commit to anything.
